Prelude and Fugue in F minor

Following Hoofdwerk's upload, Here is the Prelude and fugue in f minor.
the fugue has got a very nice melodic theme.
At the end of the fugue, some melodic lines remind the prelude. And at the very end, a choral in major mode arises majestically.
Sorry, I don't know where it comes from (who plays it on what organ ? ) I searched for the score, without result !
It's not easy to find informations about this composer. I think he deserves to be more played or published !
